Thank you for considering HALOBHID as an option to support the Individuals you serve!

Our goal is to provide individuals an opportunity to become valued and respected members of their home and community. We look forward to working with you to locate a home that best meets the needs of the Individuals on your case load.

We currently offer Sponsor Residential and Residential Services for Adults ages 18 and older with primary diagnosis of Intellectual/Developmental disability and/or dual diagnosis, that can also include mental illness (funded through the Community Living Waiver).

During the referral process HALOBHID relies on several components, which include but are not limited to:

• •Verbal and written communication with ID Support Coordinator to include the referral form
• •Current psychiatric evaluation
• •Comprehensive assessment
• •Admissions application

If the individual meets qualifications for possible placement, a tour will be scheduled for approved locations and/or the provider and will schedule an onsite visit to the individual's current location.
